ചക്കുളത്തമ്മ സഞ്ജീവനി ആശ്രമം ചാരിറ്റബിൾ ട്രസ്റ്റ്, ഡൽഹി  2021-22 അധ്യയന വർഷത്തിൽ സയൻസ്, കോമേഴ്‌സ്, ഹ്യൂമാനിറ്റീസ് എന്നീ വിഷയങ്ങളിൽ ഉന്നത വിജയം കരസ്ഥമാക്കിയ 12-ാം ക്ലാസിലെ 3 വിദ്യാർത്ഥികൾക്ക് വർഷം തോറും നൽകി വരുന്ന ചക്കുളത്തമ്മ അക്കാദമിക്  എക്സലൻസ്  അവാർഡുകൾക്കായി അപേക്ഷകൾ ക്ഷണിക്കുന്നു.

ഇതിൽ ഉൾപ്പെടുത്തിയിട്ടുള്ള അപേക്ഷ ഫാറം പൂരിപ്പിച്ചു ഫോട്ടോ, മാർക്ക് ഷീറ്റുകൾ എന്നിവ  സ്വയം സാക്ഷ്യപ്പെടുത്തി സമർപ്പിക്കേണ്ടതാണ്. മാതാപിതാക്കളുടെ പേരും ഒപ്പും അപേക്ഷാ ഫാറത്തിൽ അനിവാര്യമാണ്.

പൂർണമായും പൂരിപ്പിച്ച അപേക്ഷകൾ 9811427285 എന്ന വാട്ട്സ്ആപ്പ് നമ്പറിൽ ഒക്ടോബർ 15 വരെ  സ്വീകരിക്കുന്നതാണ്.

1. There will be three awards for class XII, will be one each for the disciplines of Science, Commerce and Humanities.
2. The Awards will be for scoring the highest marks in the respective Board Examinations.
3. The amount of each award will be decided by Chakkulathamma Sanjeevani Asramam Charitable Trust, Delhi (CASACT DELHI) In case of more than one eligible candidate the amount will be shared.
4. The award will be presented during the Chakkulathamma Ponkala Maholsavam to be held on Sunday, 30 th October 2022 organized by CASACT DELHI. 

Eligibility

1. The applicant should passed the Board Examination on first attempt during the year 2021 22 for which the award is applied for.
2. Minimum of 90 marks should be obtained in each subject.
3. Candidate should appear for the examination from a School in NCT/NCR of Delhi.
4. The application should be furnished in the proforma prescribed for this purpose by CASACT DELHI, which could be obtained from CASACT DELHI A self attested copy of the mark list and a copy of the recent photograph of the applicant should be furnished along with the application. The application should be countersigned by the parents by the prescribed date.

Selection

1. The CASACT DELHI will authenticate after proper verification of the address, etc of the parents of the applicant.
2. A committee consisting of the President, Vice President, Secretary Joint Secretary, Treasurer, Joint Treasurer and Internal Auditor will select the awardees.
3. The decision of the committee will be final and binding.
4. No correspondence will be entertained on the decision of the Committee. 

Other Condition

1. The recipient will be intimated in advance and he or she should be present at the time of the awards presentation ceremony In case the applicant is unable to receive the award in person, prior intimation about the same should be given to the Secretary, CASACT DELHI.
2. CASACT DELHI has the liberty to publish the details and photograph of the winners in social media and the newspapers or any other publications.
